I copied and pasted the letter to the email and the pages extended from originally four pages to now six pages and I also attached a second copy of the letter document to the email for download.

Demand for Accommodation sent to the private man acting as Chief Financial Officer, K. Jon Taylor and the private man acting as Chief Executive Officer, Steven E. Strah.  I sent two documents for demand for accommodation by "free mail" url postage paid to both private men.  Their response to me was silent however, my electric was shutoff the last week in June of 2021.

After uploading my Demand for Accommodation to Columbia Gas of Ohio website and sending a copy by regular mail, I continue to get Gas billing statements in the mail and on April 30, 2021 an early morning telephone message from Columbia Gas of Ohio threatening to shut off my gas to my home with a disconnect date of May 08, 2021 if I did not submit payment to them.  

I contacted a Columbia Gas of Ohio Representative and Assistant Manager, Ken, badge identification # 5169 by telephone on April 30, 2021 in response to the disconnection notice and my Demand for duty to accommodate.  Ken verified and acknowledged that he was in receipt of my four page document upload to the Columbia Gas of Ohio Website.  see document successfully uploaded to Document upload form- above on March 14, 2021.  *note, Ken explained that he was not authorized nor allowed to give his last name upon request.

Second Demand for Accommodation sent to the private woman acting as Chief Financial Officer, Kathy Shroyer of Columbia Gas of Ohio.  I sent my demand for accommodation by regular "Free Mail" to the Columbia Gas of Ohio Columbus, Ohio to Kathy Shroyer however, the mail was returned stating no such person at the address.
